Our aims and objectives

  • Through the development of services, training and development and agreed pathways access to mental health services by veterans/reservists and families are available to the standards required, when and where they are needed.
  • Through effective integrated pathways the transition of those who have been seriously injured in the course of their duty, or leave for other reasons is streamlined and continuous.
  • Through the development of communicated options of pathways, equality and ease of access for armed services personnel/veterans and their families to the services they need.
  • Through effective communication, collaboration, training and development of all partners’ within Sussex there is increase awareness of the needs of the Armed Forces Community resulting in appropriate integrated care.

To improve the years of Life for individuals whether through the treatment/support of mental and/or physical health, this could be achieved through the partnership networking providing integrated multidisciplinary care i.e. substance and alcohol misuse, homelessness, offenders, social, financial distress, mental and physical health. Individuals receive the care and support required to ensure that the outcomes result in an increase life due to improved circumstances.

To improve the Quality of Life whether through support of mental and/or physical through earlier identification and intervention i.e. the charities can provide financial support, experienced mental health interventions the quality of life could be significantly improved. This could for example prevent an individual becoming homeless (or sofa surfing), breakup of marriage and succumbing to other ailments.

To ensure integrated care positive experience for the service community through the experienced workforce with clear network of partners an individual will receive the multi-agency support required with reduced/no repetitive handover. The overall care provided ensuring a positive experience for the individual or family.

To undertake improvements in other areas which have been identified in addition to the nation requirements which are described in more detail in appendix 1. These include Joint Strategic Need Assessment, Veterans feedback and from the stakeholders of the Network. Some areas the CCGs will support the Civil Partnership Boards (Local Authorities) to deliver the community covenant and other areas will be led by the NHS.

To improve the needs identified by the individuals or identified through the above assessments by ensuring greater understanding from across Sussex. The following are the key areas where improvements could be undertaken ensuring there are clear fair processes and systems across Sussex:

  • Importance of GPs in the early identification, intervention and support.
  • Awareness, Communication(/li)
  • Single point of contact, Continuity (tell your story only once),
  • Easy Access to treatment,
  • Evidenced Based Treatments and Clear Clinical Pathways
  • Data, Identification of Veterans and Data Base (Service Directory)
  • Flexible Support Network (user group), Armed Forces Champions, Mentor for Veteran
  • Education for all (including within schools, employers, public, professionals and veterans)
    Improvements/Interventions

The interventions identified require actions to be undertaken in the following areas detailed plan is provided as part of the 2 year plan:

  • Integrated Care through multidisciplinary and multi-agency work in delivering services and support.
  • High quality urgent/emergency work – Mental Health access
  • Specialised – NHS Direct Commissioning has the responsibility for commissioning primary care and services for those in service. The CCGs will work closely with the NHS England Regional Team to ensure providers provide this care when required. In addition the CCGs will commission the community mental health and physical health for veterans, reservists (not mobilised) and families. This will be explicit in the contracts.
  • Primary Care – Though the GP element is commissioned by NHS England, the CCGs will ensure that primary care is has a raised awareness and is able to access the support/service it requires.
  • All areas within the work programme that is developed will ensure that quality, access, innovation, value for money with CCG support, evidence based practice, engagement (stakeholder, patient, public, veterans, families and reservist), choice, 7 day working and safe guarding vulnerable people are taken into account.

Measures

  • Veterans/Reservists and families experience/feedback through various routes feedback on the experience will be sought; focus groups, charities, PROMs and armed forces meeting.
  • The Charities, veterans, professionals have a system which works. The improvements identified by the veterans are implemented and the target group feel they are supported.
  • A wider group of individuals understand the culture and needs of this target group. This could be measured by the uptake of training.
  • Staff feels able to deal with situation and therefore have increased satisfaction
